To create your account, please fill out this form and then create your login on our  Provider Portal. You can access a Set Up Guide or watch this instructional walkthrough video of the registration and ordering process.

Once your account is verified, you can place patient orders. Upon registration, we will send all providers a welcome email, including a link to our comprehensive onboarding guide. This document gives you detailed information about our test, the science, and all of the logistical pieces of the process.
