### Request a GenomicMD Test

If you provide your physician’s information, we will let them know how to order a test. If we cannot reach your physician, we can provide a list of genomic-centric physicians familiar with preventative screening for consideration. Alternatively, please direct your physician to [www.genomicmd.com/register](https://www.genomicmd.com/register?hsLang=en) so they can register and, once verified, order a test.
